The 4.0.2 version is exhibiting the same issue in not being able to open existing saved form library content in the app. A new item can be created and saved, but cannot be directly reopened. This problem occurs on the on-premise SharePoint Enterprise 2013 side using claims-based authentication. The problem does not occur on our SP 2010 flavor of Office365.

To troubleshoot this further I require test credentials with steps to reproduce. You can send this information to our support inbox at support@infragistics.com. I just tested the 4.0.3 release. Same problem is occurring as described in this original post. I sent test credentials and link to your Support mailbox previously. Did you get them?
Also, I cleared the credentials cache, refreshed each site and library in the app, and also replaced the original account with a new one after upgrading to v4.0.3.
